COVID19 Safety Protocol and Liability Discliamer.

Bare Beauty Carlsbad SPA and the customers will adhere to the following Novus Coronavirus (“COVID-19”) safety protocol:

 

1. Health screening – the customer will be subject to health screening by Bare Beauty Carlsbad SPA including taking temperature and other symtomps assessment prior to entry into the PREMISES of the SPA.

2. Face covering – the customer will wear a mask or face covering when at the PREMISES of Bare Beauty Carlsbad SPA.

3. Social Distancing – the customer will practice social distancing and stay at least 6 feet from other people (except during facial treatment with esthetician) whener possible.

4. Sanitation -Bare Beauty Carlsbad SPA will disintect and clean its PREMISES, with more frequent cleaning of high touch surface.

 

Customers will ensure:

1. No Symptoms – customer have not had in the past 14 days of the following symptoms – fever, malaise, dry cough, shortness of breath or other flu-like symptoms.

2. No Travel – customer have not been out of California in the past 14 days.

3. No Contact -customer  has not been in close contact with any person who has travelled outside of the United States or with any person who is confirmed or preliminary positive case of COVID-19 infection in the past 14-days.

4. Assumption of risk – Customer is aware of the inherent risk of COVID-19 at Bare Beauty Carlsbad SPA premises and is willing to take the risk.

5. Covenant not to sue -customer promises not to take legal actions on Bare Beauty Carlsbad SPA should it be infected with COVID-19 at SPA’s PREMISES.

6. Waiver of liability – customer waives the right to hold Bare Beauty Carlsbad liable for any COVID-19 infections.

7. Indemnification – customer will indemnifiy Bare Beauty Carlsbad for any claims against the SPA relating to myself, my guest and my familiy.

8. Responsibility for Others – customers also understand that I am also responsible for the declaration of my guests and family members.

9. Confirmation – customers will aknowledge and confirm that the above information is accurate to the best of their knowledge, and agree that such information will be processed and used in accordance with Bare Beauty Carlsbad’s Privacy Policy Statement.
